Case Manager

An exciting opportunity exists for an experienced Case Manager to work alongside with our young people within our Inside Out team. Our Inside Our Program provides housing and support to vulnerable young people in the care of the Department of Health and Human Services in a lead tenant accommodation model.

The role of the Case Manager is to support and equip young people (aged 16 to 19) with life skills, building positive relationships and working towards reaching their goals and potential.

Reporting to the Manager of Transition and Support, you will embody Concern Australia’s commitment to being a child safe organisation, supporting staff and volunteers to provide high quality of service and care to young people who have experienced trauma in their lives.

For over 25 years Concern Australia has provided support and advocacy for some of the most disadvantaged and marginalised individuals and groups within the community. 

Concern Australia is in the business of transforming the lives of marginalised and vulnerable young people who society has forgotten or rejected, re-orienting young people who are growing up without any cohesive world view or value system.

We are seeking a Case Manager to provide holistic case management services and be the primary contact and liaison for our Clients’ total Care Plan, ensuring Agency requirements are met.

Applicants need to meet the following selection criteria:

·    Demonstrated commitment in line with Concern Australia’s values

·    Knowledge of the Youth Justice system and Child Protection out of home care systems

·    Experience working with young people with challenging behaviors/trauma symptoms

·    Ability to engage and support young people towards their goals

·    Demonstrated experience in working with all levels of management and external stakeholders

·    Demonstrated experience in being an efficient problem solver and a well-organized coordinator

·    Valid working with children check, satisfactory National Police Check (or international where required) and valid Australian Driver’s license

·    Tertiary qualification in either social work, youth work, community services, social sciences or other related qualification


-     Essential is a Diploma in Social Work, Community Services, Youth Work or related field.

-     Certificate IV in Training and Assessment will be highly regarded.

This is a fulltime role and an attractive salary packaging is available for the successful candidate.

The incumbent will be in possession of a current, valid Driver’s License and will be required to undergo a National Police Check (internal) and Working with Children Check.
